Step by Step Instructions - Apply for Funding
 
1. Open and Print the application form - click here to open form
 
2. Complete Part A of the form you just printed
 
3. Check all the boxes in Part B, 1
 
4. Fill in Part B, 2 with the Innovation Workshop information provided:

Course Name: Innovation Workshop

 

Name of Training Provider:  Sector Learning Solutions

 

Start/End Dates of Training: Choose the month you'd like to receive training (eg. Feb 2010)

 

Training Purchase Cost per Employee: $5000 divided by number of employees (max 12) listed in Part A (eg. for 12 staff = $416.67)

 

Description of Other Training Related Costs: none

 
5. Complete Part B, 3.
Amount Requested = # of employees x Cost per Employee (maximum in this field is $5000.00)
 
For Innovation Workshop, total amount requested is $5000.00 (up to 12 employees).  Please adjust Cost per Employee amount in previous section as needed.
 
6. Sign and Date the Form on Page 2
 
7. Mail completed application form to the address on Page 2
